We have recently introduced an improved text message reminder service. Under this system, you will receive text message reminders, sent to your mobile, to confirm and remind you of any appointments you have made. You may also receive occasional messages from us regarding particular campaigns that we may be running (e.g. seasonal ‘flu vaccinations).


If you do not already receive these messages and would like to do so, please ensure that you provide us with an up-to-date mobile number and that you remember to let us know if you change it.


If, on the other hand, you do not want to receive any text messages from us, then please just inform Reception.

LECCG EDS2 Public Survey

Lincolnshire East Clinical Commissioning Group (LECCG) are committed to delivering the Equality Delivery System 2 (EDS2). EDS2 aims to deliver better outcomes for patients and communities and better working environments for staff.

Commitment to improve patient access to their medical records

The Liquorpond Surgery makes a commitment to our patients that by 31st March 2015 we will provide the following services and systems for our patients.

Statement of Intent - GP2GP transfer of patient records.

o GP practices are obliged to make available a statement of intent regarding GP2GP. This document is the Statement of Intent from Liquorpond Surgery.

o The Government requires all practices to utilise the electronic GP2GP facility for transfer of patient records between practices, when a patient registers or de-registers (not for temporary registration)

o We will use the GP2GP facility for all transfers of patient records unless there is an issue with the receiving practice’s computer system which does not allow or provide such transfers by 21st November 2014.

o The practice clinical system is fully compliant with GP2GP.

Statement of Intent - Patient Online: Records Access

o GP practices are obliged to make available a statement of intent regarding patient on-line access to records.

o The Liquorpond Surgery is planning to offer our patients the facility to view parts of their GP medical record online by 31st March 2015.

o Patients will be able to view this information by logging onto an online account.  This will be the same online account which some patients already use to book appointments and order repeat medication.

o If you are not yet registered to use our online services, Patient Access, please come into the practice with photo ID and an account will be created for you.

Your medical record and you- Statement of Intent

o It is important that your medical record is available when and where you need it.

o You may feel, especially if you have an extensive medical history, that it is very important for some of your medical information to be available if you were on holiday away from the practice area and or taken ill and ended up in hospital.Having your Summary Care Record (SCR) available will help anyone treating you without your full medical record.  They will have access to specific information from your medical record about any medication you may be taking and any drugs that you have a recorded allergy or sensitivity to.

o Your SCR is automatically updated on at least a daily basis to ensure that your information is as up to date as it can possibly be.

o Of course if you do not want your medical records to be available in this way then you will need to let us know so that we can update your record accordingly.
