You will be asked for your name, date of birth, the item(s) you require and the dose.
Please allow 2 working days for the prescription to be ready.
Patients moving into our practice area who are not registered with a doctor in this area. We also accept patients already living in the area who wish to transfer from another surgery.
Patients no longer register with a particular doctor, but will register with the practice. There will still continue to be a doctor’s name ‘attached’ to your registration, but you are free to see any of the practice’s doctors.
Registration involves completion of a registration card and we will also require two forms of personal identification (one showing your current address and another preferably with your photograph on). If you have your old medical card, then please bring this with you as well. Please check that you live within the practice area. If you are unsure about any aspect of registration, or whether you are entitled to receive NHS services, please phone or ask at the reception as the regulations are quite complicated.

People who reside outside UK or are not eligible for free treatment through the NHS will be seen as private patients. Basic charges are as follows:
If you are an EEA national you will be asked to produce form E112 or E128 to establish your entitlement to free treatment through the NHS.
